Change of Circumstances form

If any of your details have changed since you completed your original application you MUST complete a Change of Circumstances form and send it back to us as soon as possible. Changes such as new a address, new name or adding/removing household members.

Disability and Medical form

If you or anyone in your houshold has medical conditions and they have changed you MUST let us know as quickly as possible as your priority band could change.
